Job Title: Physical Therapist Profession: Physical Therapist Specialty: N/A Duration: 13 months Shift: M-F 8a-4:30p Hours per Shift: 8 Experience: Five years of progressively responsible Physical Therapy experience. Minimum of two years Physical Therapy experience in a clinical setting with a frail or elderly population. License: Valid Physical Therapy License Certifications: BLS Must-Have: Physical Therapy knowledge and skills necessary to treat frail, elderly participants with complex needs. Description: The Physical Therapist is responsible for coordinating and providing restorative and rehabilitative Physical Therapy services to participants. The Physical Therapist works in close coordination with the Physician, rehabilitation staff, and other interdisciplinary team members to maximize participant independence and safety and develop or restore function. The Physical Therapist collaborates with the Occupational Therapist to efficiently operate the rehabilitation department. The Physical Therapist is a member of the management team and facilitates collaborative relationships across the organization and with external stakeholders to promote the organization's values, team culture, and mission. Duties include conducting Physical Therapy assessments to determine care needs, preferences, and goals of participants. The Physical Therapist actively participates in interdisciplinary team meetings to develop participant care plans. The Physical Therapist delivers and documents Physical Therapy interventions in accordance with physician orders. Interventions may include providing active and passive exercise, muscle re-education, gait, and functional training of activities of daily living. Treatment modalities may include massage, heat, sound, water, light, or electricity. The Physical Therapist supervises Therapy Assistants, providing training, instruction, and guidance needed to implement Functional Maintenance Programs. Training and instructions are provided to staff, participants, and caregivers/families in the use and care of therapeutic equipment. The Physical Therapist orders, tracks, and manages maintenance for equipment. Prompt updates to the interdisciplinary team regarding any changes in participant condition or functional status are necessary. Working with the interdisciplinary team supports smooth care transitions between settings. The Physical Therapist participates in end-of-life care coordination and support. Management of direct reports includes training, supervising, and mentoring. The Physical Therapist must be legally authorized to practice in their location and must act within the scope of their authority. All required documentation must be completed in a timely and accurate manner. The Physical Therapist protects privacy and maintains confidentiality of company procedures and participant information. WelbeHealth policies and procedures must be followed, and participation in required Quality Improvement activities, staff training, and meetings is expected. Regular communication with supervisors and team members regarding workload and priorities is essential. Timely completion of all mandated trainings and occupational health screenings is required. Flexibility in performing assignments as business needs evolve is necessary. Other duties may be assigned as needed.
