Job Title: Physical Therapist Profession: Physical Therapist Specialty: Home Health Duration: 13 weeks Shift: Monday to Friday Hours per Shift: 8:00 AM - 5:00 PM Experience: Minimum of one year of experience as a Physical Therapist preferred. License: Currently licensed as a Physical Therapist in the state. Certifications: Must have a current American Heart Association BLS for Healthcare Provider Card. Must-Have: Home Health experience and familiarity with Homecare Homebase preferred. Description: The Physical Therapist is responsible for providing prescribed treatments of physical therapy. These treatments involve a complex and sophisticated level of modalities designed to restore the patient to his or her maximum rehabilitative potential. The Physical Therapist evaluates, plans, and provides care to patients with deficits in self-care, mobility, safety, range of motion, or strength. The Physical Therapist supervises and directs the care provided by the Physical Therapy Assistant. The Physical Therapist may supervise the care provided by aides to increase the patient's functional abilities and rehabilitative potential. This position requires operation of a vehicle for the organization. The selected candidate must possess a valid, current driver's license. The Physical Therapist provides direct hands-on patient care following the patient's plan of care and current standards of practice. All care provided, coordination of services, orders, goals, etc., must be documented in the patient's electronic medical record in an accurate and timely manner. The Physical Therapist supervises care provided by the PTA and Home Health Aides to ensure treatment goals are met. Coordination with other disciplines, agencies, families, etc., is essential to ensure that the care needs of the patient are met.
