Job Title: Physical Therapist Profession: Physical Therapist Specialty: Home Health Duration: 13 weeks Shift: Monday to Friday Hours per Shift: 8:00 AM - 5:00 PM Experience: Minimum of one (1) year of experience as a Physical Therapist preferred. License: Currently licensed as a Physical Therapist in the state. Certifications: Must have a current American Heart Association BLS for Healthcare Provider Card. Must-Have: - Home Health experience - Familiarity with Homecare Homebase Description: The Physical Therapist is responsible for providing prescribed treatments of physical therapy. This involves a complex and sophisticated level of modalities designed to restore the patient to maximum rehabilitative potential. The Physical Therapist evaluates, plans, and provides care to patients with deficits in self-care, mobility, safety, range of motion, or strength. The Physical Therapist supervises and directs the care provided by the Physical Therapy Assistant. The Physical Therapist may also supervise the care provided by aides to enhance the patient’s functional abilities and rehabilitative potential. This position requires the operation of a vehicle for the organization. The selected candidate must possess a valid, current driver's license. Direct hands-on patient care must be provided according to the patient's plan of care and current standards of practice for Physical Therapy. All care provided, coordination of services, orders, goals, etc., must be documented in the patient’s electronic medical record accurately and timely. The Physical Therapist supervises the care provided by the Physical Therapy Assistant and aides to ensure that treatment goals are being met. Coordination with other disciplines, agencies, families, and others is necessary to meet the care needs of the patient.
