- Evaluate residents mobility, balance, strength, gait, and functional limitations through initial and ongoing assessments.
- Develop and implement individualized, person-centered physical therapy treatment plans to improve function, reduce pain, and prevent disability.
- Deliver direct therapy interventions, including therapeutic exercises, gait training, balance retraining, and application of physical modalities.
- Monitor resident progress, reassess as needed, and adjust treatment plans to achieve therapy goals.

- Support quality assurance and performance improvement initiatives within the rehab team.
- Work in a Skilled Nursing Facility (SNF)/Long-Term Care setting, requiring frequent standing, bending, lifting, and hands-on resident mobility assistance.
- Collaborate effectively within a fast-paced, team-based rehabilitation environment.
