- Evaluate residents' mobility, balance, strength, gait, and functional limitations through initial and ongoing assessments.
- Develop and implement individualized, person-centered treatment plans to restore/improve physical function, reduce pain, and prevent disability.
- Deliver direct therapy interventions such as therapeutic exercises, gait training, balance retraining, and use of modalities/physical agents.
- Monitor residents progress and adjust care plans to ensure therapy goals are met.
- Collaborate with interdisciplinary team members, participate in care team meetings, and assist with discharge planning.
- Educate residents, families, and caregivers on home exercise programs and safety strategies to promote independence.
- Supervise and guide Physical Therapist Assistants (PTAs) and therapy aides in accordance with licensure requirements.
- Accurately document treatment sessions, progress notes, and billing information in a timely manner.
- Ensure compliance with state, federal, and facility documentation and practice standards.
- Support quality assurance and performance improvement initiatives within the rehab team.
- Work in a skilled nursing facility (SNF) or long-term care setting, which may require frequent standing, bending, lifting, and resident assistance.
- Maintain strong clinical, communication, and organizational skills, working independently and as part of a team.
