Job Description:
We are seeking a compassionate and skilled Physical Therapist (PT) to join our interdisciplinary team in a Skilled Nursing Facility (SNF). The Physical Therapist is responsible for evaluating residents' physical conditions, developing personalized treatment plans, and delivering rehabilitative care to help patients improve mobility, restore function, and achieve the highest possible level of independence.
Responsibilities:
- Perform thorough assessments of patients' physical abilities, movement limitations, and pain levels.
- Develop and implement individualized treatment plans based on evaluation findings, medical history, and patient goals.
- Deliver evidence-based physical therapy interventions to improve strength, coordination, balance, and mobility.
- Collaborate with physicians, nursing staff, occupational and speech therapists, and family members to coordinate patient care.
- Monitor patient progress, adjust therapy plans as needed, and document all care provided in accordance with state and federal guidelines.
- Educate patients and caregivers on safe mobility techniques, home exercises, and equipment use.
- Ensure compliance with all regulatory requirements including Medicare/Medicaid documentation and facility policies.
- Participate in care planning meetings and interdisciplinary team conferences.
- Supervise physical therapist assistants (PTAs) and aides as needed.
- Maintain a safe and clean work environment.
Requirements:
- Graduate of an accredited Physical Therapy program.
- Current state license to practice as a Physical Therapist (PT).
- CPR certification (preferred or required depending on the facility).
- Experience in a SNF or geriatric rehab setting preferred.
- Excellent communication, documentation, and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
