Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in South Carolina required to start the assignment. Candidates with current licensure in additional states listed by the client are acceptable.
• Current CPR certification.
• Valid driver's license, vehicle insurance, and access to a dependable vehicle for travel within the agency service area.
• Experience providing home health physical therapy, including evaluation, plan-of-care development, interventions to improve function and mobility, and documentation consistent with CMS and agency policy.
• Ability to complete evaluations and submit outcomes, goals, and projected frequency of care within 24 hours of evaluation.
• Schedule flexibility to participate in weekend rotation and potential availability on observed holidays per agency needs.
• Preferred: Experience supervising PTAs and home health aides per state regulation and agency policy.
