Responsibilities
- Conduct clinical assessments to identify patient needs and determine appropriateness for therapy; develop, implement, and update a Plan of Care (POC) and progress notes, communicating with the physician and interdisciplinary team.
- Plan and deliver physical therapy for neuromuscular, neurological, and orthopedic conditions based on medical referrals and ongoing evaluation data; adjust treatment as needed.
- Instruct patients and families in transfer techniques appropriate to patient abilities; educate on the safe use of equipment such as walkers, wheelchairs, canes, braces, and prosthetic devices; modify equipment to meet home and patient needs.
- Maintain comprehensive documentation including POC, progress notes, phone orders, and other clinical records; review and revise the POC as assessments evolve.
- Promote infection control and universal precautions; provide patient/family education tailored to age, condition, culture, and learning readiness.
- Collaborate with an interdisciplinary team and, when applicable, supervise other team members in delivering patient care aligned with patient goals.
Requirements
- Active Physical Therapist license – Required
- CPR certification – Required
- Current driver's license with reliable transportation and insurance – Required
- 1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting – Required
- Bachelor of Science in Physical Therapy from an APTA-approved program – Required
