We are seeking a highly skilled Clinical Oncology Pharmacist to support hematology/oncology infusion services in an inpatient/acute care setting. This role focuses on clinical dosing, oncology infusion therapies, and medication management for complex patient populations.
Key Responsibilities- Perform clinical oncology pharmacy services including dosing and therapeutic monitoring
- Manage chemotherapy and infusion-related medications
- Conduct pharmacokinetic dosing and medication adjustments
- Prepare and compound IV medications, including IV piggyback and IV push
- Monitor patient response and optimize medication therapy
- Collaborate with interdisciplinary teams to ensure safe and effective care
- Utilize Epic EMR for documentation and order verification
- Ensure compliance with all safety, regulatory, and hospital protocols
