Candidate Pay Rate: $90-$95 per hour

Roles & Responsibilities- Provide specialized oncology infusion pharmacy care, ensuring safe and effective medication dosing for oncology patients
- Perform clinical dosing and pharmacokinetic dosing with attention to therapeutic medication monitoring
- Prepare and verify compounding, IV piggyback and IV push medications with rigorous safety checks
- Document care and medication interventions accurately in Epic and other charting systems
- Collaborate with interdisciplinary teams to optimize treatment plans and improve patient outcomes
- Support operational needs including walking between clinic locations when needed to ensure continuity of care
- Participate in block scheduling and holiday coverage planning as part of the clinical team
- PharmD required
- Licensed Pharmacist, State of CA (Pharmacist State of CA)
- Minimum 2 years recent Oncology Clinical Infusion experience (required)
- Epic experience (required) and comfort with electronic charting systems
- BLS certification preferred
- RPh credential and familiarity with medication dispensing systems (Baxter experience a plus)
- Ability to float/walk between locations when needed and work collaboratively with sister facilities
- Comfort with inpatient/acute care environments and sterile compounding practices
- PharmD (Doctor of Pharmacy) – required
- Inpatient / Acute care medication management
- Clinical dosing and pharmacokinetic dosing expertise
- Therapeutic medication monitoring best practices
- Compounding techniques and IV preparation (IV piggyback, IV push)
- Experience with Epic and electronic charting systems
- Familiarity with medication dispensing systems and Baxter automation
- Strong clinical judgment and interdisciplinary communication skills
