Position Summary:
The Occupational Therapist (OT) is responsible for evaluating residents, developing individualized treatment plans, and providing evidence-based occupational therapy interventions to improve functional independence and quality of life. The OT collaborates closely with interdisciplinary team members, residents, and families to support optimal rehabilitation outcomes within the skilled nursing setting.

Key Responsibilities:
  • Evaluate residents functional abilities, including ADLs, mobility, cognition, and psychosocial factors.
  • Develop and implement individualized treatment plans based on evaluation findings and physician orders.
  • Provide direct occupational therapy services in accordance with state and federal regulations and facility policies.
  • Monitor and document resident progress, adjusting treatment plans as necessary to meet goals.
  • Participate in care planning meetings and collaborate with the interdisciplinary team.
  • Educate residents, families, and caregivers on strategies to promote safety and independence.
  • Recommend adaptive equipment and assistive devices as needed.
  • Supervise and provide direction to Certified Occupational Therapy Assistants (COTAs) and therapy aides, as appropriate.
  • Ensure accurate, timely, and compliant documentation in the electronic medical record.
  • Maintain licensure and continuing education requirements as mandated by state regulations.
  • Support quality improvement initiatives and comply with CMS and facility guidelines.
Qualifications:
  • Graduate of an accredited Occupational Therapy program.
  • Current Occupational Therapist license in the state of practice.
  • Prior SNF or geriatric rehabilitation experience preferred.
  • Strong clinical reasoning, communication, and organizational skills.
  • Ability to work effectively in a team-oriented environment.
