Job Title: Home Health Occupational Therapist Profession: Therapy/Rehabilitation Specialty: Occupational Therapy Duration: 20 Weeks Shift: Day Hours per Shift: 8 Experience: Minimum 1-2 years of experience required License: Valid state license required Certifications: BLS, state licensed Must-Have: - Home health experience required - OASIS training preferred - Must be an independent practitioner - Must have a valid driver's license and reliable transportation for home visits - Must be COVID vaccinated or have an exemption form on file - Must have a flu shot or declination Description: Provide skilled home health occupational therapy services. Deliver care to patients with a variety of conditions including med/surg, cardiac, orthopedic, and respiratory issues. Perform home visits, typically 6-8 per day, ensuring a minimum of 6 points daily at manager’s discretion. Utilize skills in activities of daily living, cardiac rehab, edema massage, feeding techniques, splinting, and more. Engage in patient and family education regarding safety and injury prevention. Complete standardized assessments and develop individualized home exercise programs. Conduct evaluations, reassessments, and discharge planning while documenting care accurately. Support patients recovering from surgery or coping with chronic conditions like COPD, CHF, and Parkinson's disease. Work collaboratively with an interdisciplinary team to provide comprehensive patient care. Adhere to all safety and infection control protocols while conducting home visits.
