The RN Case Manager OB/GYN is responsible for coordinating and managing care for patients within the OB/GYN clinic, ensuring timely, safe, and patient-centered services across the continuum of care. This role collaborates closely with providers and interdisciplinary teams to develop individualized care plans, support clinical decision-making, and improve outcomes through proactive case management and follow-up. The RN Case Manager also performs direct clinical nursing duties in the clinic, serving as a key point of contact for patients and families.

Actively manage OB/GYN patients and coordinate care plans across clinic visits, procedures, and follow-up.
Collaborate with physicians, advanced practice providers, and interdisciplinary staff to establish and monitor individualized medical goals.
Serve as the primary clinical contact for patient care coordination, questions, and education.
Facilitate, track, and follow up on referrals to surgery and specialty clinics, helping reduce delays and barriers to care.
Perform staff nursing duties including patient assessments, documentation, education, and medication administration within scope.
Identify and troubleshoot barriers to care, including scheduling issues, referral bottlenecks, and patient access challenges.
Maintain clinical competencies related to safety, infection control, and evidence-based nursing practice in the OB/GYN setting.
Active Registered Nurse (RN) license (Alaska or compact; Alaska licensure application required prior to start).
Graduate of an accredited school of nursing; BSN preferred.
Minimum of 1 year of clinical nursing experience (OB/GYN or women s health experience preferred).
Current BLS certification through the American Heart Association (AHA).
Demonstrated case management and care coordination skills in an ambulatory or acute care setting.
Excellent communication, problem-solving, and critical-thinking abilities.
Strong organizational skills with the ability to manage multiple priorities in a fast-paced clinic environment.
