Job Title: Hospice Registered Nurse (RN) Profession: Nursing Specialty: Hospice RN Duration: 13 weeks Shift: Days Hours per Shift: 32 Experience: Minimum of 3 years nursing experience, preferably in hospice, home health or palliative care. License: Current Registered Nurse (RN) license in the state or compact state RN license. Certifications: Current BLS Certification. Must-Have: - Strong clinical assessment skills. - Excellent communication, interpersonal, and organizational skills. - Compassionate demeanor and ability to support patients and families during difficult times. - Valid driver's license and reliable transportation required for home visits. Description: We are seeking a dedicated and compassionate Hospice Registered Nurse (RN) to join our interdisciplinary team. The RN will provide skilled nursing care and support to terminally ill patients and their families in accordance with the patient's plan of care and hospice philosophy. Key responsibilities include performing comprehensive assessments of patients' needs and coordinating individualized care plans. The RN will provide pain and symptom management consistent with the hospice plan of care. Education and support regarding the disease process, self-care techniques, end-of-life issues, and the hospice philosophy will be provided to patients and families. Collaboration with the interdisciplinary team including physicians, social workers, chaplains, and volunteers is essential. Participation in on-call rotation is required. Accurate and timely documentation in compliance with relevant policies must be maintained. On-call requirements include 1-3 shifts per week, including 3 weekends during the contract. Response to patient needs within 45 minutes when on call is expected. Holiday coverage will be required. Experience with EMR software is preferred.
