Job Title: Hospice Liaison Profession: Operations Specialty: Home-Hospice Care Operations Duration: 13 weeks Shift: 10hr Day Shift Hours per Shift: 40 Experience: Minimum of 2 years required; hospice liaison experience preferred but open to other hospice experience. License: BLS, Driver's License Certifications: Auto Insurance, Vehicle Must-Have: EPIC experience required. Comfortable with symptom management for end-of-life, including pain and shortness of breath. Competent in educating providers on managing end-of-life medication. Capable of speaking and collaborating with physicians. Understanding of hospice benefits and philosophy. Knowledge of different levels of hospice care, including general inpatient, routine, and respite service. Description: Responsibilities include monitoring queue and consulting with patients referred for hospice. Provide assessments and recommendations for the proper hospice setting for patients transitioning from a hospital. Admit patients to hospice services in a hospital setting as needed. May require travel to multiple hospitals, with assignments based on size and patient census. Focus on adult patients with diagnoses such as heart failure, cancer, and other end-of-life conditions. Support is readily available from supervisors, managers, and a liaison team for any questions. Technology used includes EMR systems for charting and secure communication. Floating may be necessary, requiring travel to 1-2 hospitals per day. Work schedule is Monday through Friday from 8:00 AM to 6:00 PM, with one rotating day off each week. Business casual dress or professional scrubs are acceptable; a white coat is preferred but not required.
