Job Title: Home Health Registered Nurse Profession: Registered Nurse (RN) Specialty: Home Health Duration: 13 weeks Shift: Days Hours per Shift: 8 Experience: 2+ years Nursing experience, 6 months Home Health experience, Travel experience License: Active and clear driver's license, Hawaii license Certifications: COVID and Flu vaccine Must-Have: - Homecare Home Base and OASIS Description: Provides excellent patient care through collaboration, development, implementation, revision, and reporting of the case management program. Acts as a liaison between the patient, family, nurse, physicians, multidisciplinary team, and the patient's healthcare benefactor to optimize outcomes. Serves as a consultant to the healthcare team on specific patient items. Leads the coordination of patient care with other disciplines within the care team while monitoring the appropriateness and timeliness of care. Ensures the interdisciplinary care plan aligns with the patient's clinical course, continuing care needs, and covered services by monitoring diagnostic testing, treatments, and procedures. Engages in discussions with physicians regarding the appropriateness of resource utilization, consultations, treatment plans, estimated length of stay, and discharge plans. Focuses on complex patients, frequent emergency department utilizers, chronic pain, substance abuse patients, and homeless patients. Collaborates with acute care case managers to ensure the appropriateness of ongoing care. Coordinates the transfer of patients to tertiary centers, including transferring necessary patient information for continuity of treatment and services. Provides oversight and collects data required for regulatory and accreditation compliance. Manages frequent visitors to the emergency department by conducting focused study reviews of previous and current admissions.
