Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice Experience depending on the specialty. License: Must have a valid license in hand. Certifications: CPR, BLS Must-Have: Must be proficient in OASIS and have used it within the last 6 months, or complete a retraining course during compliance. All travelers must be proficient in HomeCare HomeBase EMR and have used it within the last 6 months, or complete a retraining course during compliance. Must be open to traveling up to 75 miles from the assigned care center. Must be able to accept every patient on the schedule regardless of distance from the previous appointment. Must understand that there are no guaranteed hours with this contract. Documentation must be completed within 24 hours of the appointment. Must be open to on-call rotations. Description: Provides and directs the nursing care in home settings according to physician orders. This role is based on agency policies and procedures utilizing the nursing process competently. Responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Home visits and periodic office meetings are required. The position demands reliable transportation, a current driver’s license, and agency-required liability insurance. On average, the nurse will see 5-8 patients per day. Weekly productivity expectations are 30 points, with each visit and activity carrying different point values. Various types of cases include Start of Care, Wound Care, and Follow-Up Visits. First-time travelers are not accepted for this role; recent specialty experience is required. Weekend requirements vary based on location and profession. The contractor will be compensated based on patient home visits. The daily job involves completing documentation efficiently, as delays can impact billing and follow-up patient care. The nurse must be comfortable with longer drives between patients, sometimes exceeding 20 minutes. The incumbent must carry out responsibilities with professionalism and adherence to agency standards.
