Job Title: Home Health & Hospice Registered Nurse Profession: RN (Registered Nurse) Specialty: Home Health & Hospice Duration: 13 weeks Shift: Day Shift Hours per Shift: 8 hours, 8a-5p Experience: Minimum 1 year of Home Health & Hospice experience License: Current Driver's License and Auto Insurance required Certifications: BLS Certification required Must-Have: Home Health & Hospice experience Description: Provide care within a 60-mile radius, including homes, nursing homes, and various patient settings. Engage in initial assessments of clients and families to determine home health needs. Complete physical assessments and histories of current and previous illnesses. Regularly re-evaluate client nursing needs based on health evaluation data. Initiate and revise plans of care as client status and needs change. Develop care plans and establish goals based on nursing diagnoses. Incorporate therapeutic, preventive, and rehabilitative nursing actions. Involve clients and families in the planning process. Administer medications and treatments as prescribed by physicians. Provide health care instructions based on assessments and care plans. Identify discharge planning needs as part of care plan development before discharge. Act as Case Manager when assigned, coordinating client care for assigned caseload. Prepare clinical notes and update primary physicians as necessary. Communicate with physicians regarding client needs and report changes in condition. Ensure necessary equipment and services are arranged for client care. Instruct, supervise, and evaluate care provided by home health aides. Commit to weekend and holiday coverage as part of the on-call rotation. Complete required duties within the scope of home health and hospice care.
