Job Title: Hospice RN, Home Health RN Profession: Nursing Specialty: Hospice RN, Home Health RN Duration: 13 weeks Shift: Schedule varies Hours per Shift: 40 Experience: Minimum 2 years License: Current RN license in the State of Montana (BSN preferred) Certifications: Valid driver's license, appropriate auto insurance, and reliable transportation required Must-Have: - 2 years of recent acute care, home care, or hospice experience - Clinical skills such as venipuncture, catheterization, wound care, and/or parenteral therapy preferred Description: Provide skilled nursing, care, and case management for assigned patients in their place of residence or in an in-patient Hospice Center. Admit patients onto services. Coordinate and integrate services to ensure continuity of care. Facilitate the achievement of quality, clinical, and cost outcomes. Negotiate, procure, and coordinate services and resources needed by the patient or family. Create opportunities and systems to enhance outcomes. Utilize the nursing process of assessment, planning, and implementation of care, including patient and family instruction and evaluation through an interdisciplinary team approach. Provide skilled nursing and coordinate care to patients and family members in a residential setting through a team approach and based upon systematic evaluation as authorized by a physician. Observe, assess, evaluate, and plan interventions in relation to patients' physical and psychosocial statuses, including changes in condition and/or reactions to treatments or medications for the duration of contact with patients. Oversee care provided by Home Health/Hospice Aides and CNAs to assigned patients and evaluate compliance with the plan of care. Assure continuity of care through effective communication, information exchange, and case conferences with other disciplines and community health, support, and social agencies. Provide quality care and appropriate symptom management for positive clinical outcomes. Promote optimal health and quality of life through teaching, counseling, appropriate interventions, and referrals. Maintain accurate, current, and complete patient records in accordance with agency standards and OASIS requirements. Participate in team scheduling coordination and on-call rotation schedule as necessary to meet patient care needs by assessing and evaluating patient needs via telephone and advising or making home visits as required. Participate in quality improvement activities and attend in-service training as assigned. Perform other duties as assigned by the supervisor.
