Job Description:

The RN in the Home Health department will provide care within a 60-mile radius around Dickinson, including patient homes and nursing facilities. The role requires initial assessments, care planning, administering medications, and coordinating care as a Case Manager.

Responsibilities:

• Initial assessment of client and family to determine home health needs, including a complete physical assessment and history.
• Regularly re-evaluate client nursing needs and use health assessment data to determine nursing diagnosis.
• Initiate and revise care plans based on client status and needs, incorporating therapeutic, preventive, and rehabilitative actions.
• Administer medications and treatments prescribed by the physician.
• Provide health care instructions to clients and identify discharge planning needs as part of care plan development.
• Act as Case Manager and assume responsibility to coordinate client care for assigned caseload.
• Prepare clinical notes, update primary physicians, and ensure arrangements for equipment and other necessary items and services.
• Instruct, supervise, and evaluate home health aide care provided.

Requirements:

• BLS Certification
• Home Health & Hospice experience required.
• Current Driver's License & Auto Insurance
