Position Summary
The Registered Nurse will assess the health of individuals served, implement and supervise care plans, and promote health maintenance and disease prevention. This role may also involve case management and staff supervision.
Key Responsibilities
- Observe, assess, plan, implement, and evaluate nursing support for individuals with acute, chronic, or wellness needs in community or small facility settings.
- Collaborate with healthcare providers to plan, implement, and evaluate programs addressing client health needs.
- Coordinate communication and actions across disciplines to ensure health and safety supports.
- Manage medication processes: ordering, reviewing, monitoring, discontinuation, and disposal per physician orders.
- Ensure policies on medication administration, errors, storage, and destruction are reviewed annually and updated as needed.
- Report changes in client condition to physicians; transcribe physician phone orders and medical reports.
- Complete required nursing documentation (daily, monthly, quarterly, annual).
- Maintain accurate nurse’s notes on individual health status.
- Ensure furnishings, equipment, and supplies meet infection control standards.
- Supervise licensed practical/vocational nurses as needed.
- Schedule and follow up on health-related appointments, implementing appropriate responses to findings.
- Communicate with inpatient nursing and discharge coordinators during hospitalizations.
- Provide new employee training and annual recertification on health and safety topics.
- Train unlicensed staff on person-specific health maintenance, safety, and disease prevention needs.
