Job Title: Weekend Field Nurse Profession: Nursing Specialty: Wound Care, Home Health Nursing, Hospice Nursing Duration: 13 weeks Shift: Saturday & Sunday Hours per Shift: 7:00 am - 11:00 pm Experience: Minimum of one year working with the frail, elderly, or long-term care population. License: RN Nursing Diploma or Degree from an accredited School of Nursing or University. Certifications: Current CPR certification or willingness to obtain. Must-Have: Bachelor’s degree preferred in the Science of Nursing. Minimum of one year home care experience preferred. Minimum of one year wound care experience preferred. Possess reliable transportation, a valid driver’s license, and car insurance. Must be 18 years of age or older. Must be fully vaccinated or willing to be fully vaccinated against specified communicable diseases. Description: The primary purpose of this position is to conduct participant assessments to determine appropriate levels of care. This role requires prompt responses to communications from primary care providers and support staff. Regular field visits will be performed to deliver essential nursing care, including wound care and health assessments. Coordination with various stakeholders is essential to ensure comprehensive participant support. Documenting interactions and initiating emergency treatments as needed is a key responsibility. Engagement in quality improvement initiatives is part of the responsibilities. Adaptability to changing workloads is necessary while maintaining effective communication with leadership. Participant assessments will be performed in person to meet medical needs safely. Emergency, urgent, and non-urgent home care status must be evaluated. Calls from primary care providers should be responded to promptly. Document all calls and contacts and follow up as required. Complete field visits utilizing full body assessment skills, critical thinking, and EMR. Home visits will be made regularly to provide care and treatment as ordered. This includes wound care, IV hydration, lab draws, nurse assessments, and other nursing tasks. Education, instruction, and support must be provided to participants, family, and facility staff. Direct coordination with providers, staff, and suppliers is necessary. Emergency treatment should be initiated if required. Document new orders as indicated and update necessary information. Complete appropriate documentation for reported incidents and quality measures. Collaboration with all stakeholders involved in participant care is essential. Active participation in employee orientation and ongoing training is required. Adjustments may be needed based on changes in team priorities. Keeping leadership informed of any issues is important. Other duties may be assigned as necessary.
