Key Responsibilities of the SNF RN:
- Provide high-quality nursing care to residents in a skilled nursing/long-term care setting
- Perform thorough assessments and monitor changes in resident conditions
- Administer medications and treatments according to facility protocols
- Collaborate with physicians, therapists, and interdisciplinary care teams
- Support ADLs, care planning, and family communication
- Maintain accurate and compliant documentation within the EMR system
- Ensure resident comfort, dignity, and safety at all times
Requirements:
- Active West Virginia RN license (or ability to obtain prior to start)
- BLS required
- Minimum 2 years of recent SNF, LTC, or Rehab RN experience
- Strong assessment and communication skills; experience with geriatric care preferred
