RN Case Manager Hospice

Salary/Pay Rate: $45/hour and up (based on experience)
NurseAmerica is seeking a compassionate and organized RN Case Manager to join our hospice care team. This role offers autonomy and flexibility, allowing you to manage your own daily schedule while delivering exceptional end-of-life care to patients and families.
The RN Case Manager plays a vital role in coordinating care, managing a caseload of hospice patients, and ensuring comfort, dignity, and support throughout the care journey.
Key ResponsibilitiesManage a caseload of hospice patients receiving end-of-life care
Assess and evaluate patient needs using the nursing process, with strong focus on pain management and symptom control
Deliver skilled nursing care in accordance with individualized plans of care
Educate and counsel patients, families, and caregivers regarding hospice services and care plans
Assist with admissions as needed
Provide guidance on Medicare, Medicaid, and other third-party reimbursement programs
Participate in back-up on-call rotation (one night per month)
Cover two holidays per year
Document care accurately in the electronic health record system
Active Registered Nurse (RN) license State of Maryland
Minimum 1 year of acute care, home health, or hospice experience
Strong knowledge of hospice and palliative care practices
Excellent communication and customer service skills
Detail-oriented, compassionate, and highly organized
Proficient in Microsoft Office 365 and electronic health record (EHR) systems
Ability to work independently while collaborating with a multidisciplinary team
Reliable transportation for home and facility visits throughout Montgomery County
Competitive pay: $45/hour and up
