Key Responsibilities of the Travel Case Manager Nurse:
- Coordinate and oversee the patient's continuum of care across various healthcare settings
- Conduct comprehensive assessments and develop individualized care plans

Requirements of the Travel Case Manager Nurse:
- Active New Yoek Registered Nurse (RN) license
- NY PRI Certification
- Current BLS certification
- 1-2 Years Acute Care Discharge Planning Experience
