Position Purpose:
The Care Manager develops, assesses, and facilitates complex care management activities for members with primarily physical health needs. This role ensures high-quality, cost-effective healthcare outcomes through personalized care plans, member education, and coordination with providers and community resources.

Key Responsibilities:

  • Evaluate member needs, barriers to care, and social determinants of health, prioritizing what the member identifies as most important.

  • Develop and update individualized care plans and collaborate with providers, specialists, and community resources to address unmet needs.

  • Identify problems or barriers to care and implement appropriate care management interventions.

  • Coordinate between members, caregivers, and the provider team to ensure adequate, person-centered care.

  • Monitor member status, progress toward goals, and changes in condition, revising care plans as necessary.

  • Provide resource support to members and care managers, including employment, housing, independent living, and other services.

  • Facilitate timely access to care or services through collaboration with providers or specialists.

  • Conduct outreach via telephone, digital platforms, home visits, or other sites to assess member needs.

  • Document all care management activities accurately to ensure compliance with state, federal, and payer regulations.

  • Educate members and caregivers on disease processes, care gaps, treatment plans, referrals, and healthcare benefits.

  • Provide feedback to leadership to improve care quality and delivery in a cost-effective manner.

  • Perform additional duties as assigned.

Education/Experience:

  • Bachelor’s degree in Nursing (BSN) from an accredited institution.

  • 2–4 years of related experience in nursing or care management.

Licensure/Certification:

  • RN – Registered Nurse with State Licensure or Compact State Licensure.

Skills & Competencies:

  • Strong critical thinking and assessment skills.

  • Excellent communication and collaboration abilities.

  • Knowledge of care coordination, case management, and social determinants of health.

  • Ability to work independently and as part of a multidisciplinary team.

  • Proficiency in documentation and compliance with healthcare regulations.
