Responsible for care coordination and care transitions planning throughout the acute care patient experience. This position collaborates with the physician, utilization manager, medical social worker, and bedside RN. The goal is to assure the timely progression and transition of patients to the appropriate level of care. Prevent unnecessary admissions or readmissions through effective resource coordination. Work to achieve optimal health, access to care, and appropriate utilization of resources. Balance patient self-determination while coordinating care in a timely and integrated manner. Collaborate with patients, families, physicians, the interdisciplinary team, nursing management, quality, and ancillary services. Engage with third-party payers, review agencies, claims and finance departments, medical directors, and community resources. If assigned to the emergency department, address complex clinical and social situations efficiently. Job Accountabilities: Conduct initial and continued patient assessments. Review initial physician admission care plans. Gather additional medical, psychosocial, and financial information from patient interviews and medical records. Determine moderate or high risk level for readmission. Conduct screenings for ancillary supportive services, including palliative care needs. Functionally supervise and lead the healthcare team in developing comprehensive care coordination plans. Identify and refer quality and risk management concerns for patient safety reporting. Direct and oversee case management assistants to determine preferences for post-acute care services. Utilization Management: Review medical records to ensure patients meet level of care requirements. Confirm necessary documentation with attending physicians. Expedite transition planning for patients no longer requiring acute care. Monitor length of stay and manage outliers requiring additional resources. Collaborate with financial counselors regarding delivery of inpatient stay denials. Ensure delivery of necessary patient information prior to discharge. Actively participate in patient rounds and collaborate with interdisciplinary teams for timely transitions. Document consistently in electronic health records and maintain current knowledge of regulatory requirements. Effectively follow up on patients requiring re-evaluation for admission or transition. Care Coordination/Care Transitions: Formulate transition plans based on available care options in collaboration with patients, families, and the healthcare team. Screen for 30-day readmissions and review previous hospital records. Identify barriers to care progression and work with multi-disciplinary teams to resolve issues. Review and modify plans of care as necessary, ensuring timely transitions to lower levels of care. Communicate follow-up appointment needs to patients and families prior to transition. Facilitate paperwork for post-acute transfers in compliance with regulations. Proactively identify high utilizers in emergency departments and make appropriate referrals. Address complex psychosocial issues and coordinate with the healthcare team for support services. Department Operations: Contribute to ongoing operations by identifying new systems, processes, and protocols. Ensure cost-effective practices that enhance patient experiences and outcomes. Communicate effectively with care management colleagues to facilitate transitions. Manage all communications and actively participate in departmental meetings. Utilize effective interpersonal skills to promote customer service. Develop and maintain professional relationships with the healthcare team and community representatives. Foster a collaborative and respectful environment free of judgment. Engage in department operations with a commitment to providing and accepting constructive feedback. Utilize communication skills with colleagues to resolve issues promptly.
