Travel Nurse RN - Case Manager - $3,000 per week in Atlanta, GA

We are seeking an experienced RN Case Manager with a strong background in acute care to coordinate and manage patient care throughout the hospitalization continuum. The RN Case Manager will assess patient needs, develop and implement individualized care plans, coordinate with physicians and interdisciplinary teams, facilitate discharge planning and transitions of care, and ensure compliance with clinical, regulatory, and payer requirements.
