License: Current RN License (New York) Certifications: BLS Required Must-Have: Certified Home Health Care Experience, OASIS Experience, EPIC or Netsmart Preferred, Telemetry Experience Description: Develops and revises plans of treatment and discharge in conjunction with the patient, family, physician, and care delivery team. Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition according to defined time frames. Processes orders and notifies physician of patient needs and changes in condition. Provides an environment that promotes respect for patients' privacy, confidentiality, and property. Initiates appropriate preventive and rehabilitative strategies. Provides services requiring specialized nursing skills. Consults with and educates the patient, family, and team members regarding disease processes, self-care techniques, and preventive strategies. Supervises Licensed Practical Nurses to ensure patient health, safety, and compliance with the plan of care. Directs, coordinates, evaluates, and supervises the quality of patient care services provided. Interacts with patients, physicians, referral sources, and others to maintain positive relationships. Supports care management philosophy, including collaborative planning and case management to meet individual health and service needs. Ensures completion of all clinical records for compliance with legislation and delivery of seamless service within established timeframes. Maintains current knowledge of available services and community resources. Demonstrates flexibility and willingness to visit patients based on need. Ensures problem resolution at the point of contact. Ensures authorization and release of information forms are signed by the patient or responsible party. Attends patient care conferences for assigned cases. Monitors results and takes corrective action on quality indicators, including customer satisfaction. Understands the impact of nursing functions on patient care, other providers, readmission, productivity, and costs. Pursues efforts to reduce avoidable costs and errors. Responds to findings from Quality Management reviews and ensures resolution of potential problem areas. Participates in peer review and quality assurance as assigned. Maintains productivity requirements. Ensures appropriate visit authorizations are in place and that documentation reflects medical necessity for in-home care. Promotes recognition, respect, and celebration of diversity among the workforce. Participates in education, training programs, and self-development as needed. Demonstrates commitment to professional growth and competency. Responsible for Key Performance Indicators established by management. Performs other duties as requested.
