Job Opportunity – Hospital Case Manager – California

Start Date: ASAP
Location: Mission Hills, California
Compensation: Details provided to qualified applicants
Schedule: Weekend rotation required to meet department needs; patient ratio 1:20; 40-hour orientation provided

Requirements:

  • Active California RN license (pending licenses accepted if active by start date)
  • Minimum 2 years of acute care hospital Case Manager experience performing care coordination and discharge planning
  • Proficiency with InterQual Criteria and DRG (Diagnosis Related Groups)
  • Experience with Epic charting system required
  • Strong knowledge of care coordination, discharge planning, and plan of care development
  • Competency in arranging DME, SNF placements, and ambulance transport independently
  • Proficient in Microsoft applications
  • Experience working with diverse patient populations including adolescents, adults, and geriatrics
  • Travel healthcare experience preferred; first-time travelers considered with clinical screening
  • Minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting
