Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: Minimum of 2 years in acute care case management/discharge planning License: CA state license required; pending licenses accepted but must be licensed upon start date Certifications: InterQual guidelines knowledge preferred Must-Have: Ability to organize and participate in family meetings; experience with HMOs, IPAs, Medicare, and MediCAL Description: This position requires care coordination and discharge planning. The case load may include managing up to 25 patients. Experience in assessing patients and determining medical necessity is essential. Knowledge of adoption/foster care, substance abuse treatment options, and mental health crisis management is critical. The role will include participating in the development of treatment plans. The candidate must be competent in arranging durable medical equipment and skilled nursing facility transport. A commitment to working every other weekend and availability for holidays is required. Effective communication and teamwork are necessary for working with interdisciplinary support teams. Candidates should have experience with computerized documentation. Understanding CMS guidelines and HIPAA regulations is essential for this position. The ability to float between facilities within a given state may be required. References from supervisory roles covering at least one year in a comparable specialty are necessary.
