Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: Minimum of 2 years in Acute Care Case Management/Discharge Planning License: CA state license required; pending license accepted but must be licensed upon start date Certifications: Relevant nursing certifications Must-Have: Hospital experience; Behavioral health experience does not apply Description: Start date: ASAP Floating expectation: No First-time traveler: No Days patient ratio: 1:25 Weekends patient ratio: 1:45 Required experience: Must have hospital experience; behavioral health experience does not apply Weekend requirement: Will rotate to meet department needs (every other weekend) Previous charge experience preferred Experience with assessing patients Care coordination and discharge planning are essential Ability to organize and participate in family meetings Participate in the development of treatment plans Experience with HMOs and IPAs is beneficial Knowledge of adoption/foster care and drug/alcohol abuse treatment options is necessary Familiarity with patients experiencing homelessness and mental health crises is important Competent in arranging durable medical equipment and transport services Must perform tasks independently Experience with computerized documentation is required Daily caseload: Up to 25 patients Interdisciplinary support includes physical therapy and social services Special procedures or unit details require skilled assessment and care coordination Candidates missing required skills or experience should not be considered Commitment to working 3 of 5 holidays is necessary
