Registered Nurse - Case Management

Location: Seattle, WA

Start Date: 04-20-2026

End Date: 07-20-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 4:30 PM Day (5x8-hour day shift). Note: Minimum 2 weekend days per month required (typically 1 Saturday and 1 Sunday in separate weekends); may float to nearby Swedish campuses (Issaquah, First Hill, Ballard) as needed.

Hours per Week: 40

Candidate Pay Rate: $64 per hour

Roles & Responsibilities
  • Coordinate comprehensive case management for inpatient acute care patients, ensuring safe and timely transitions of care
  • Lead discharge planning efforts, collaborating with interdisciplinary partners like PT, pharmacy, social services and respiratory therapy
  • Manage care coordination for neuro and cardiac patients, supporting the needs of a comprehensive stroke center
  • Perform needs assessments and order DME when appropriate to support patient recovery at home
  • Document accurately in Epic to maintain clear, compliant patient records and care plans
  • Communicate proactively with patients, families, and community providers to remove barriers to care and improve outcomes
  • Float to other campuses as needed while maintaining high standards of patient-centered care
Qualifications
  • Valid Washington RN license (required)
  • BLS certification (required)
  • Minimum 2 years of inpatient/acute care experience with case management (accepted: 1st time travelers who have inpatient acute care experience)
  • Epic charting system experience (required)
  • Comfortable floating to nearby campuses and providing weekend coverage as needed
  • Experience coordinating care for neuro/cardiac patients and familiarity with stroke center workflows preferred
Knowledge & Skills
  • Strong case management and care coordination skills
  • Expertise in inpatient acute care workflows and discharge planning
  • Proficiency with Epic electronic health record
  • Ability to collaborate with interdisciplinary teams (PT, pharmacy, social work, respiratory)
  • Comfort managing patient volumes and ratios typical of acute hospital case management
  • Critical thinking, clear clinical judgment, and excellent communication skills
