Job Title: Registered Nurse Case Manager Profession: Nursing Specialty: Case Management Duration: 13 weeks Shift: Monday to Friday Hours per Shift: 8 hours Experience: Minimum one year of experience as a clinical nurse in a hospital is required; nine months of experience in home care is preferred. License: Current RN license in New York required. Certifications: BLS required. Must-Have: Certified Home Health Care experience required. Experience with OASIS assessments required. Knowledge of EPIC or Netsmart systems preferred. Description: Develop and revise treatment and discharge plans in conjunction with patients, families, physicians, care delivery teams, and third-party payors. Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition according to defined time frames. Process orders and notify physicians of patient needs and changes in condition. Complete certification, recertification orders, and discharge summaries. Provide an environment that promotes respect for patients, their privacy, confidentiality, and property. Initiate appropriate preventive and rehabilitative strategies. Provide services requiring substantial specialized nursing skills. Consult with and educate patients, families, and team members regarding disease processes, self-care techniques, and preventive strategies. Supervise LPNs to ensure patient health, safety, and compliance with the plan of care. Direct, coordinate, evaluate, and supervise the quality of patient care services provided by the Home Health Agency. Interact with patients, physicians, referral sources, and others in a manner that fosters positive relationships. Support case management philosophy through collaborative planning and service provision designed to meet individual health and service needs. Ensure completion of all appropriate clinical records for compliance with legislation and delivery of seamless services. Maintain current knowledge of available community-based services. Demonstrate flexibility and willingness to visit patients based on need rather than geography. Ensure problem resolution at the point of contact. Attend patient care conferences on patients assigned to your care. Monitor results, progress, and take corrective action on quality indicators, including customer satisfaction. Understand the impact of the RN function on patients and care providers. Pursue efforts to reduce or eliminate avoidable costs and errors. Respond to findings of Quality Management reviews and audits, ensuring resolution of potential issues. Participate in peer review and Quality Assessment and Performance Improvement efforts. Maintain productivity requirements. Ensure appropriate visit authorizations are in place and that documentation reflects medical necessity for care in the home setting. Promote recognition and respect for the diversity of the workforce. Participate in education, orientation, in-service training, and self-development as needed. Demonstrate commitment to professional growth and competency. Perform other duties as requested.
