Job Opportunity – Case Management RN – Northern California
Start Date: ASAP
Location: Santa Rosa, California
Compensation: Details provided to qualified applicants
Schedule: Full-time; 2 weekends every 6 weeks; no call required; 40-hour orientation
Highlights:
- Epic charting system utilized facility-wide
- Caseload of approximately 20 patients
- Floating between like units based on departmental needs

Requirements:
- Active RN license in California
- Minimum 2 years of case management experience required
- Current work history within the last 2 years (hard requirement)
- BLS certification required
- Epic charting system experience required
- Experience with discharge planning, care coordination, and prior authorizations
- Competency arranging DME, SNF placement, and ambulance transport independently
- Experience with HMOs, IPAs, and utilizing InterQual criteria preferred
- Knowledge of ICD-10 coding, DRG systems, and HIPAA guidelines preferred
- Community hospital, LTAC, and Trauma Level I/II experience preferred
- Previous travel experience required
