Job Opportunity – Case Management RN – Northern California

Start Date: ASAP
Location: Santa Rosa, California
Compensation: Details provided to qualified applicants
Schedule: Full-time; 2 weekends every 6 weeks; no call required; 40-hour orientation

Highlights:

  • Epic charting system utilized facility-wide
  • Caseload of approximately 20 patients
  • Floating between like units based on departmental needs
  • Comprehensive interdisciplinary support including IV teams, physical therapy, respiratory services, social services, and rapid response teams

Requirements:

  • Active RN license in California
  • Minimum 2 years of case management experience required
  • Current work history within the last 2 years (hard requirement)
  • BLS certification required
  • Epic charting system experience required
  • Experience with discharge planning, care coordination, and prior authorizations
  • Competency arranging DME, SNF placement, and ambulance transport independently
  • Experience with HMOs, IPAs, and utilizing InterQual criteria preferred
  • Knowledge of ICD-10 coding, DRG systems, and HIPAA guidelines preferred
  • Community hospital, LTAC, and Trauma Level I/II experience preferred
  • Previous travel experience required
