Job Opportunity – Case Manager/Discharge Planner – California

Start Date: ASAP
Location: Apple Valley, California
Compensation: Details provided to qualified applicants
Schedule: Days with weekend rotation (every other weekend); Patient ratio 1:25 on weekdays, 1:45 on weekends

Requirements:

  • Active California RN license required by start date (pending license acceptable during application process)
  • Minimum 2 years of acute care case management and/or discharge planning experience
  • Hospital-based experience required; behavioral health-only experience does not qualify
  • Previous travel nursing experience strongly preferred
  • Epic charting system experience required
  • Expertise in care coordination, utilization review, discharge planning, and patient assessment
  • Proficiency with HMOs, IPAs, and managed care processes
  • Independent competency arranging DME, SNF placements, and ambulance transport
  • Knowledge of community resources including foster care, substance abuse treatment, homelessness services, mental health crisis intervention, and advance directives
  • Ability to organize and facilitate family meetings and contribute to interdisciplinary treatment planning
  • Up to 7 days PTO available; must work 3 of 5 major holidays
