Job Title: Case Manager Registered Nurse Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 hours Experience: Minimum of 1 year in inpatient case management preferred. License: Active RN license required. Certifications: CPI certification preferred. Must-Have: - Experience with complex cases including guardianships and legal processes. - Ability to work with interdisciplinary teams. - Strong communication skills for coordinating care and facilitating safe discharges. Description: SW preferred with inpatient case management experience. Registered nurse with inpatient case management experience required. Agreeable to work with guardianships, capacity, and certificate of competency. Will work with a legal team and attend court hearings with patients. Experience in managing complex cases, including contacting family members and friends. Responsible for coordinating medical home care, placement, and addressing social issues. Providing wrap-around services for safe discharges. Average daily census of 15 cases managed. Holidays are required as scheduled.
