RN Case Manager

Location: 287 McLaws Circle, Williamsburg, VA 23185

Period: February 23, 2026 – April 30, 2026

Schedule: Monday–Friday, 8:00 AM – 4:30 PM (some on‑call required)

Hours: 40 per week

Weekly Pay Package:     

Weekly Tax Free Stipend to be paid    $1,324.68

Weekly Straight Pay (Taxable)  $1,200.00

Weekly Gross   $2,524.68

Shift & Compensation

• Mileage reimbursement: $0.43 per mile
• Counties covered: Williamsburg, Yorktown, Gloucester, Hampton Roads, Poquoson, Charles City
  • Current Virginia RN license or Compact RN license

License Requirements

Preferred Skills

• Hospice experience (required)
• HCHB (Homecare Homebase) experience (preferred)
• Primary Responsibilities
• Conduct comprehensive assessments of high‑risk members
• Collaborate with primary care providers to implement individualized care plans
• Identify and reassess cost‑efficient, appropriate levels of care
• Arrange non‑medical support (housing, transportation) to ensure treatment compliance
• Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals
• Engage specialty resources to achieve optimal patient outcomes
• Maintain detailed records of clinical, functional, and fiscal outcomes
• Promote health and illness prevention opportunities
• Intervene quickly to minimize adverse patient outcomes
• Serve as patient advocate, ensuring privacy and confidentiality
  • Associate’s or Bachelor’s Degree in Nursing or related field

Qualifications

• Current, unrestricted RN license (Virginia or Compact)
• Case Management certification
• Strong ability to analyze complex medical information and make sound decisions
