Primary Responsibilities

  • Conduct comprehensive assessments of hospice patients.
  • Collaborate with primary care providers to develop and implement individualized care plans.
  • Continuously reassess patient needs to ensure appropriate, cost-efficient levels of care.
  • Arrange non-medical support services (housing, transportation) to ensure treatment compliance.
  • Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals.
  • Engage specialty resources as needed to achieve optimal patient outcomes.
  • Maintain detailed records of clinical, functional, and fiscal outcomes.
  • Identify opportunities for health promotion and illness prevention.
  • Intervene quickly to prevent adverse patient outcomes when possible.
  • Serve as a patient advocate, ensuring privacy and confidentiality.

 

Qualifications

  • Associate’s or Bachelor’s Degree in Nursing or related field.
  • Current, unrestricted RN license in the state of Minnesota.
  • Proven ability to analyze complex medical information and make sound clinical decisions.
  • Hospice experience is required.
