Work Location: 303 Quartermaster Ct, Jeffersonville, IN 47130
Hours: 8 hours/day, 40 hours/week
Shift: Monday–Friday, 8:00 AM–5:00 PM (Weekend on-call rotation)
Mileage Reimbursement: $0.46 per mile
Counties Covered: Harrison, Crawford, partial Floyd, Washington
Weekly Pay Package

Primary Responsibilities
- Conduct comprehensive assessments of high-risk members.
- Collaborate with primary care providers to implement individualized care plans.
- Ensure cost-efficient, appropriate levels of care; reassess continuously.
- Arrange non-medical support (housing, transportation) to improve compliance.
- Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals.
- Engage specialty resources as needed for optimal outcomes.
- Maintain detailed records of clinical, functional, and fiscal outcomes.
- Identify opportunities for health promotion and illness prevention.
- Intervene quickly to prevent adverse patient outcomes.
- Advocate for patients, ensuring privacy and confidentiality.
Qualifications
- Associate’s or Bachelor’s Degree in Nursing or related field.
- Current, unrestricted RN license.
- Case Management certification required.
- Strong analytical skills to interpret complex medical information and make sound decisions.
