Job Title: Registered Nurse Case Manager Profession: Registered Nurse Specialty: Case Manager Duration: 6 months Shift: Day Hours per Shift: 8 Experience: Minimum of 3 years in a nursing role License: Must have state RN license in hand Certifications: BLS, ACM or CCM Must-Have: - Care coordination - Discharge planning - Knowledge of HIPAA guidelines Description: Conducts an initial patient and family assessment on every inpatient to determine psycho-social needs and discharge plans. Completes Care Coordination Assessment within the first 48 hours of admission to determine the need for Care Coordination intervention. Performs documentation on a weekly basis in patients' medical charts. Documentation includes significant patient data, defined problems, assessment goals, treatment plans, actions taken, and follow-up activities as needed. Increases documentation frequency if the patient’s situation changes. Participates as a member of the Multidisciplinary Rounds. Serves a consultative role with staff regarding patients and families. Communicates clinical status updates to patients, families, and other involved individuals. Regularly evaluates the appropriateness of the treatment plan concerning the patient’s progress toward stated goals. Communicates evaluations with the patient and/or family. Reviews final discharge plans and instructions with the patient, family, or responsible party, with documentation in the patient's medical chart. Maintains a record for each patient, noting discharge date and any services the patient was referred to. Documents this information in the medical record. Responsible for discharge planning for the patient and family. Coordinates the discharge plan with the entire rehabilitation team.
