This role is responsible for the collaborative practice of assessment, planning, facilitation, care coordination, evaluation, and advocacy. The goal is to meet individual and family comprehensive healthcare needs. Effective communication and resource utilization are essential to promote patient safety and quality care. The case manager addresses the needs of patients after critical events or diagnoses requiring complex management strategies. Services are provided for patients from various healthcare settings, including ambulatory and inpatient. Key responsibilities include: Conducting comprehensive assessments of patient and family needs. Developing personalized patient-centered care plans. Engaging patients and families as members of the care team. Utilizing professional judgment and critical thinking to assist patients in overcoming barriers. Providing counseling related to treatment decisions and end-of-life issues. Ensuring patients transition seamlessly between care settings. Advocating for appropriate service delivery within the health plan structure. Collaborating with care team members to co-manage patients with complex needs. Evaluating patient responses to care plans and making necessary modifications. Participating in process improvement activities to reduce risk. Facilitating interventions in cases of abuse or neglect. Supporting departmental goals that contribute to organizational success. Other duties may be assigned as necessary.
