Job Title: Case Manager Profession: RN Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8:00 AM - 5:30 PM Experience: Minimum 2 years License: RN, LCSW, PT, SLP, OT Certifications: CCM, ARN, ACM or CM Certifications preferred Description: We are seeking a skilled Case Manager to join our dedicated team. The facility is a state-of-the-art rehabilitation hospital. We provide therapy services to individuals who have experienced a loss of function due to injury or illness. Our rehabilitation programs offer ongoing care and specialized treatment tailored to individual needs. We assist those recovering from various conditions, including stroke, brain injury, neurological conditions, trauma, spinal cord injury, amputation, and orthopedic injury. Our goal is to maximize health, function, and quality of life through comprehensive rehabilitation programs. We are accredited for our Stroke Specialty, Comprehensive Integrated Inpatient Rehabilitation, and Amputation Specialty Programs. The ideal candidate will have strong clinical skills, the ability to work collaboratively with the rehabilitation team, and a passion for improving patient outcomes.
