Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 14 weeks Shift: Day Hours per Shift: 8:30 AM - 5:00 PM Experience: 2 years RN experience License: Active RN License Certifications: BLS Must-Have: Current in-patient Case Management experience Experience with EPIC and post-acute facility referral platforms such as NaviHealth or AllScripts Description: The Case Manager coordinates care for a select caseload of patients from triage through care coordination and safe transfer to home. They proactively communicate with payors to ensure approval of treatment plans and required resources. The role requires maintaining knowledge of clinical systems and fiscal data related to specific patient populations. Collaboration with the multi-disciplinary team and patients/families is essential to ensure effective transition planning. The Case Manager coordinates transitions to post-acute settings, including nursing facilities, acute inpatient rehab, and local shelters. They manage patients based on departmental procedures while maintaining clinical safety and quality. Patient information must be entered and maintained in all departmental systems per operational guidelines. Please note that current/active CPR certification is required for this role.
