Job Description:

Responsibilities include performing case management duties such as discharge planning, patient assessment, patient and family conferences, and collaborating with physicians to ensure a safe discharge plan. The role involves working with skilled nursing facilities, home health, hospice, and long-term acute care hospitals.

Responsibilities:

• Rounding on patients that meet high risk criteria for discharge planning needs
• Working with MD and Nurses in arranging medication placement and transportation

Requirements:

• Knowledge of discharge planning, patient assessment, and conducting patient family conferences
• Experience working with physicians on safe discharge plans and coordinating with skilled nursing facilities, home health, hospice, and LTACH
• Meditech experience preferred
