Description: This is a priority start. We may be able to get approval for a conditional start pending onboarding. In this role, the successful candidate will support the coordination, integration, communication, implementation, and evaluation of nursing practice and clinical policies and procedures. The Ambulatory Care Nurse will participate in site and organizational quality and performance improvement activities. The candidate will collect patient healthcare data systematically using appropriate assessment techniques and instruments. Collaboration with licensed providers to develop patient-centered plans of care that prescribe interventions to attain expected outcomes is essential. The Ambulatory Care Nurse will provide comprehensive care in support of all patients' requirements. This includes the administration of treatments and medication, acting as a sentinel for untoward events or symptoms, disease prevention guidance, and supportive care for symptom relief. Responsibilities also include performing physical examinations and diagnostic tests. The candidate will collect patient health histories and record information in the electronic health record. Providing information, counsel, and education to patients and families regarding test results and treatment plans as directed is crucial. The role involves administering medications including vaccination, IV therapy, and oxygen administration. The candidate will provide comprehensive wound care and perform phone and in-person patient triage for clinical concerns. Assessing and monitoring patient responses to various treatments, procedures, and anesthesia is required. The candidate will assess and interpret patient information, making treatment decisions as appropriate. Consultation with clinical supervisors and licensed providers to determine the best treatment plans for patients is expected. The role includes directing, supervising, and delegating to licensed practical nurses, medical assistants, and technicians as needed to support patient flow and efficiency. The candidate will perform other duties as needed and ensure clinical compliance with patient safety initiatives and reporting. In addition, the Ambulatory Care Nurse will support key quality initiatives to impact clinical outcome measures. Participation in designated activities related to clinical standards and regulations of relevant outside agencies is necessary. The candidate will support mission, philosophy, standards, goals, and objectives of the organization. The role includes collaboration with members of the interdisciplinary team and promoting collegial relationships. Acting as a support and resource for nursing, other clinical staff, and providers is critical. The candidate will participate in clinical inquiry and foster evidence-based practice. Engagement in self-assessment and participation in activities to promote professional growth and development is expected. The candidate will seek, acquire, and maintain current nursing knowledge and competence to fulfill responsibilities. Implementation of medical regimens as prescribed by licensed providers is required. Assessment of patients to identify unmet needs and plan comprehensive care is essential. Integration of care with other healthcare providers such as therapists and social workers is expected. Providing direct care services, including but not limited to administration of treatments and medication, is required. This role involves collecting and documenting patient clinical historical and assessment data. Onsite testing and medical treatments will be performed in accordance with the plan of care. The candidate will assist with minor and major procedures and monitor patients before, during, and after procedures. Medication administration will be done as per provider orders or protocols, including oxygen therapy. Communication of critical lab values and follow-up instructions to patients as directed by the provider is necessary. Care and case management of a defined population via multi-disciplinary plan to coordinate patient care is expected. Identifying patient deviations from health and wellbeing and initiating appropriate actions will be required. The candidate will educate patients and families to facilitate and promote healthy adjustments to new or changed health patterns. Evaluation of patient progress toward expected outcomes is also part of the role. The Ambulatory Care Nurse will strive to drive consistency in every patient and colleague encounter.
