Join our team as the Director of Care Management under the North Division Executive Director of Care Management. Oversee care management for two hospitals (390 beds and 128 beds combined), leading teams to ensure seamless patient care from admission to discharge, advocate for patients, and drive financial viability through optimal resource use and next-site-of-care coordination.

Key Responsibilities:

  • Direct and facilitate care management processes for top-tier patient care and efficiency.

  • Champion continuity of care, patient advocacy, and resource management to reduce fragmentation.

  • Foster collaboration and communication across healthcare teams.

  • Promote cultural and religious sensitivity in all patient interactions.

Essential Functions:

  • Align team with organizational Mission, Vision, and Core Values.

  • Develop customer satisfaction programs.

  • Recruit, coach, and manage high-performing staff.

  • Ensure compliance with infection control, risk management, confidentiality, and regulations.

  • Drive cultural competency, safety, and security protocols.

  • 2+ years as RN in acute care.

  • 3+ years managing care coordination (utilization review knowledge preferred).

  • Proven independent leadership and systems thinking for problem-solving.
