Roles & Responsibilities
  • Manage a caseload of approximately 60–65 HCBA Waiver Program participants with compassion and organization
  • Conduct comprehensive health and psychosocial assessments to identify needs and risks
  • Develop and implement individualized Plans of Treatment (POT) in collaboration with participants, families, and clinical staff
  • Coordinate transfer and discharge planning and arrange timely authorizations for services
  • Provide community-based services via in-home visits, telehealth, and telephonic follow-up, including monthly monitoring
  • Monitor service delivery and quality, performing reassessments and annual face-to-face visits
  • Serve as an advocate and liaison to community agencies, ensuring participants access appropriate resources
  • Maintain accurate, timely case management documentation and utilize EHR systems effectively
Qualifications
  • Master's Degree from an accredited School of Social Work (MSW required)
  • Minimum 2 years combined experience in any of: managed care health plans; utilization review, case management, or discharge planning; or transitional/acute care settings
  • Valid California driver’s license, access to an insured vehicle, and ability to drive within San Diego County (and possibly Imperial County)
  • T.B. clearance and ability to lift up to 50 lbs as needed for role requirements
  • Strong Microsoft Office skills (Word, Excel, PowerPoint) and computer data entry/analysis experience
  • Experience with Electronic Health Records; Epic experience preferred
  • Demonstrated cultural sensitivity, strong communication, organizational skills, and clinical judgment
  • Reliable, self-starting, and able to prioritize a large volume of work to meet deadlines
Education
  • Master's Degree from an accredited School of Social Work (MSW)
Knowledge & Skills
  • Case management and care coordination for high-risk and vulnerable populations
  • Assessment, development, implementation, and monitoring of Plans of Treatment (POT)
  • Discharge planning, utilization review, and transitional care processes
  • Proficient documentation and use of EHR systems (Epic preferred)
  • Strong Microsoft Office proficiency for reporting and analysis
  • Excellent communication, advocacy, and community liaison skills
  • Knowledge of HMO and waiver program regulations and community resources
  • Time management, critical thinking, and customer-focused service skills
