Job Title: Social Services Practitioner Profession: Social Services Practitioner Specialty: Social Services Practitioner Duration: 13 months Shift: Day Shift Hours per Shift: M-F 8am-5pm Experience: 2 years in clinical/social work License: Required Certifications: Preferred Must-Have: - Bachelor's Degree or equivalent - Master's Degree in social work or equivalent combination of education/experience Description: Renders clinical psychosocial services to patients and their families of all ages. Assesses patients' psychosocial needs and makes referrals to appropriate facilities or community agencies. Provides crisis intervention and bereavement counseling. Utilizes effective problem-solving strategies to reach decisions and take appropriate action. Provides consultation and counseling with patients, families, staff, and others in the service of patients. Participates in departmental and interdisciplinary meetings and activities. Obtains or coordinates resources from community, social, health, and welfare agencies. Provides training for nursing staff to intervene during on-call hours and weekends.
