Job Title: Inpatient Case Management Social Worker Profession: Social Services Specialty: LMSW (Licensed Masters of Social Worker) Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:30 - 17:00) Experience: Minimum of 1 year of acute care experience License: LMSW or LCSW (pending license accepted) Certifications: AK License as LMSW or LCSW Must-Have: - Acute care experience Description: The Inpatient Case Management Social Worker provides psychosocial and emotional support to patients and families. Social Work is a consult-based service. Clinicians complete psychosocial assessments and identify appropriate discharge plans from a person-centered approach and in alignment with policy and regulations. Social Work clinicians provide brief clinical interventions as appropriate, assisting with resource navigation and coordination of care. They screen for suicidality and engage in safety planning as appropriate and complete reports of harm based on state statutes. Clinicians coordinate with outside entities as needed. Social Work is divided into various teams focusing on different patient populations. Clinicians work closely with Case Managers and the care team to coordinate and execute safe and ethical discharge plans. The role involves a variety of patient diagnoses and requires a broad skill set in social work practices. Candidates should be prepared to accommodate weekend work. Travelers may float within the hospital or to nearby affiliate facilities as necessary. The role may involve interaction with a diverse patient population, emphasizing respect and cultural competence in care delivery.
